- Name of sponsor (Sponsors must be adults over the age of 18 who are willing to accept responsibility for the supervision of their students during the tournament.)
- Sponsor’s e-mail address
- Sponsor’s phone number
- Approximate number of students sponsor is bringing to the tournament
