Do you want to be part of transforming the way primary health care is delivered to the most vulnerable citizens in Washington State?  WACMHC is currently recruiting for a Practice Improvement Coordinator to take an active role in promoting the Patient Centered Medical Home model in Washington’s Community Health Centers.  This mission-driven professional will coach at the individual clinic level as well as facilitate sharing of best practices across the state.  This announcement may be used to fill multiple openings.

WACMHC, a non-profit founded in 1985, advocates for access to quality primary health care for underserved populations and underserved geographic areas in Washington State through its 26 member Community Health Centers (CHCs).  WACMHC staff support Washington’s Federally Qualified Health Centers (FQHCs) by providing training and technical assistance as well as advocating on behalf of members at the state and federal level.

WACMHC member Community Health Centers provide comprehensive services including medical, dental and behavioral health care in addition to enabling services in medical interpretation, transportation, assistance with enrolling in Medicaid or other health insurance, health education/promotion, nutrition and access to other social services.  FQHCs are a key part of the social safety net and accept patients regardless of their insurance status or ability to pay.  For more information, visit http://wacmhc.org

Position Summary

The Practice Improvement Coordinator is the key contact between the Community Health Centers and WACMHC to provide training, technical assistance and coaching to clinic staff in the areas of improving clinical outcomes, implementing the Patient Centered Medical Home and continuing to advance Meaningful Use of electronic medical records.  The Practice Improvement Coordinator will assess CHC needs and identify experts and resources to bring to the CHCs.   S/He will provide direct coaching and facilitate the sharing of best practices between community health centers. S/He will be a member of the DOH Washington Health Improvement Network (WHIN) Team that runs regionally based collaboratives promoting clinical quality and best practices in the primary care setting.

Essential Duties

  • Develop and sustain relationships with quality improvement staff at the Community Health Centers
  • Continuously assess and respond to CHC needs for training, technical assistance and coaching in areas related to clinical quality, achieving Meaningful Use or implementing the Patient Centered Medical Home (PCMH) model
  • Provide expert NCQA PCMH documentation reviews for CHCs seeking NCQA PCMH recognition
  • Coordinate quarterly WACMHC Quality Improvement Roundtables for CHC quality directors including identifying content experts, facilitating and evaluating the learning sessions
  • Participate as a full team member of the Department of Health (DOH) Washington Health Improvement Network (WHIN)
  • Coach staff of  CHCs participating in DOH WHIN Regional Collaboratives
  • Coordinate, manage and distribute clinical and quality improvement information and resources to Washington CHCs, partners and stakeholders through a quarterly newsletter.
  • Develop and deliver high quality webinar presentations
  • Develop and administer surveys using Survey Monkey
  • Support CHC submission of data to DOH to track quality improvement goals and initiatives
  • Provide and manage content for WACMHC’s quality improvement web pages
  • Assists with other WACMHC projects as needed and available

Qualifications

Required Education

Bachelor’s Degree in nursing, health administration, health informatics, or other related field OR more than three years work experience in a healthcare setting.

Required Experience, Knowledge, Skills and Abilities

  • Minimum two years experience in a health related field
  • Demonstrated ability to communicate orally and in writing to groups and one-on-one
  • Excellent customer service skills
  • Demonstrated organizational skills
  • Intermediate level mastery of Microsoft Office applications.
  • Demonstrated ability to work independently and as part of a team

Preferred Experience, Knowledge, Skills and Abilities

  • Knowledge of PCMH standards and Meaningful Use
  • Knowledge of using data to drive quality improvement in a primary care setting
  • Strong excel skills
  • Experience developing and administering surveys using Survey Monkey
  • Experience in meeting event planning and facilitation
  • Commitment to the Community Health Center mission
  • Willingness to travel in state

Preferred Certification

  • NCQA Certified Content Expert in Patient Centered Medical Home or willingness to attain within six months of employment at the employer’s expense
